Understanding the OAT Axis

Balanced hormones, a healthy menstrual cycle, and optimal fertility hinge on the harmonious function of the ovaries, adrenal glands, and thyroid. Collectively, these form the ovarian-adrenal-thyroid (OAT) axis.

Disruptions in any part of this axis can lead to a spectrum of health issues, including:​

  • Menstrual irregularities (e.g., heavy or irregular bleeding, cramping, PMS)

  • Infertility and miscarriage

  • Low libido

  • Weight fluctuations

  • Persistent fatigue

  • Cognitive challenges (e.g., brain fog, poor concentration)

  • Mood disturbances (e.g., depression, irritability)

  • Skin and hair concerns (e.g., dryness, hair loss)

  • Digestive symptoms

  • Sugar cravings

  • Muscle or joint pain​

The Roles of the OAT Axis Components

Ovaries: Situated in the pelvis, the ovaries regulate the reproductive cycle through hormone production. In a typical menstrual cycle, oestrogen peaks during the follicular phase (pre-ovulation), while progesterone dominates the luteal phase (post-ovulation).​

Adrenal Glands: Located atop the kidneys, the adrenal glands respond to stress by producing hormones such as adrenaline, noradrenaline, and cortisol.​

Thyroid Gland: Found in the neck, the thyroid regulates metabolism via hormones T4 (thyroxine) and T3 (triiodothyronine). These hormones influence cellular energy production and are integral to the synthesis of sex-hormone binding globulin, prolactin, and gonadotropin-releasing hormones, all vital for menstrual health and fertility.​

The hypothalamus in the brain oversees each of these glands, ensuring their interdependent functions maintain hormonal balance.​

The Impact of Chronic Stress

A significant disruptor of the OAT axis is chronic stress – be it physical or psychological. 

  • Continuous busyness

  • Physical discomfort or pain

  • Emotional distress

  • Sleep deprivation

  • Excessive exercise

  • Nutrient deficiencies

  • Inadequate food intake

  • Infections

  • Alcohol consumption

  • Exposure to heavy metals​

Chronic stress initiates a hormonal cascade, often leading to prolonged elevated cortisol levels. An analogy: if the thyroid is the engine, the adrenals are the fuel tank, and cortisol is the fuel. An excess of fuel (cortisol) can flood the engine (thyroid), impairing its function.

Elevated cortisol can reduce active T3 hormone availability, slowing metabolism and energy production. Moreover, high cortisol levels can diminish progesterone production, leading to hormonal imbalances such as oestrogen dominance.​

Recognising OAT Axis Dysregulation

Individuals may experience dysfunction in one or multiple components of the OAT axis:​

  • Thyroid Dominance: Symptoms include profound fatigue and signs of hypothyroidism.

  • Adrenal Dominance: Commonly presents with low energy, anxiety, and irritability.

  • Ovarian Dominance: May manifest as brain fog, memory issues, and pronounced PMS symptoms.​

Addressing OAT Axis Imbalances

To restore balance within the OAT axis:

  1. Comprehensive Assessment: Identify which components of the OAT axis are affected.

  2. Holistic Approach: Address all interconnected systems rather than isolating a single issue.
